Forms and Procedures for Officials:

  • Adverse Weather Conditions Procedures:  Officials shall evaluate the situation with a SAFETY FIRST approach.  A guideline for resuming a game after a lighting/thunderstorm is to wait 30 minutes after the last visible sign of lightening and/or audible sound of thunder.  School regulations and guidelines governing the waiting time will take precedence.

  • Ejection Procedures:  Any time an ejection occurs (coaches or players ejection) during a game, the official making the ejection must file an OSLA Ejection Report.  This report is to be completed and e-mailed by noon of the next business day following the game or contest.  The report is sent to the ejected person's athletic administrator/director and the OSLA Commisisioner.  Click here for the form.

  • Red Card Procedures:  Umpires are required to submit the OSLA Suspension Card Report for each RED CARD issued.  This report must be submitted on the day of the game.  Click here for the form.

  • Concussion Procedures:  If there is an concussion during a game, an official must complete the OSLA Concussion Report.  This form documents that a player has been removed from a game in accordance with U.S. Lacrosse and OSLA Rules due to exhibiting signs, symptoms and/or behaviors consistent with a concussion.  The official should complete this form and forward it to OSLA within 48 hours after the conclusion of the game.  It is recommended that the official keep a copy of this report.  Click here for the form.
